1. Agreement to Services

By accessing or using the services of Tidewater Family Dentistry ("the Practice"), operated by Dr. James F. Dollar, DDS, PC, you agree to be bound by these Terms and Conditions. If you do not agree to these terms, please do not use our services.

2. Dental Services

Tidewater Family Dentistry provides general and restorative dental services including but not limited to:

• Routine dental examinations and cleanings
• Preventive dental care
• Fillings, crowns, bridges, and dentures
• Restorative dental procedures
• Other general dentistry services

All services are provided by licensed dental professionals in accordance with Virginia state regulations.

3. Appointments and Cancellations

We request that you provide at least 24 hours' notice if you need to cancel or reschedule an appointment. Repeated no-shows or late cancellations may result in a fee or require advance payment for future appointments.

Our practice will make reasonable efforts to remind you of scheduled appointments via phone, text, or email.

4. Payment and Insurance

Payment is due at the time of service unless other arrangements have been made in advance. We accept various payment methods and will file claims with your insurance provider as a courtesy.

Please note that you are responsible for any amounts not covered by your insurance, including deductibles, co-pays, and services not covered by your plan.

5. Patient Responsibilities

As a patient of Tidewater Family Dentistry, you agree to:

7. Limitation of Liability

While we strive to provide the highest quality dental care, all medical and dental procedures carry inherent risks. Tidewater Family Dentistry shall not be liable for any complications, outcomes, or damages arising from dental treatment, except in cases of gross negligence or willful misconduct.

8. Emergency Care

For dental emergencies during office hours, please call us immediately at (757) 320-9780. For after-hours emergencies, please proceed to your nearest emergency room or call our emergency line.

9. Changes to Terms

We reserve the right to modify these Terms and Conditions at any time. Changes will be posted on this website with an updated "Last modified" date. Your continued use of our services after such changes constitutes acceptance of the new terms.
